Job Title: Advanced Practice Professional
Location: York, PA
Employment Type: Full-time
What You’ll Champion:
- Monday-Friday, 8:00 AM - 4:30 PM schedule with no evenings, weekends, or holidays
- Join a collaborative team of 6 Nephrologists and 3 Advanced Practice Professionals
- Shared weekday call rotation for dialysis centers
- Manage adult nephrology patients ages 19+ in outpatient, dialysis, and inpatient settings
- Perform dialysis rounding and chronic disease management at three outpatient dialysis centers and WellSpan York Hospital
- Diagnose and treat Chronic Kidney Disease (CKD), Acute Kidney Injury (AKI), ESRD, hypertension, electrolyte disorders, anemia, and post-transplant kidney conditions
- Develop and manage your own outpatient nephrology patient panel
- Provide in-basket coverage and collaborate with physicians, APPs, nurses, and dialysis teams
- Travel between DaVita St. Charles Way, DaVita Roosevelt Avenue, U.S. Renal Care Kenneth Road, and York Hospital; mileage reimbursement provided
- Dialysis and nephrology experience preferred, but strong new graduate NPs with dialysis RN experience are encouraged to apply
- Acute Care or Critical Care experience is a plus
