About this role

POSITION OVERVIEW

Responsible for assessing client’s needs and capabilities, and for counseling and assisting clients to ensure their overall well-being and treatment for a smooth integration into the community. Establishes mental and medical health linkages and utilizes available resources in the community to help clients attain their goals.

REPORTING STRUCTURE

There are to staff reporting to this position.

SPECIFIC RESPONSIBILITIES

- Transport clients to appointments as necessary

- Prepare daily contact sheets and monthly progress notes for each client.

- Serve as liaison between clients and mental health professionals, client’s family, therapist, community agencies, care providers, and others involved in the process of problem resolution and on-going cased management.

- Perform crisis intervention and assistance as necessary.

- Attend regularly scheduled staff meetings to address concerns, questions and supervision issues.

- Hold meetings monthly or as needed with clients to address any concerns that they may have; coordinate scheduling of client appointments as needed.

- Develop Individualized Rehabilitation Plan for each client; conduct annual interdisciplinary planning meeting and review with each client on a semi-annual basis.

- Seek housing placements for clients as needed, and monitor quality and adequacy of housing

- Monitor adequacy of, and assist clients in accessing and managing entitlements.

- Communicate with other supportive staff in regards to client’s concerns

- Provide referral, intake, and orientation processes for new clients.

- Maintain quality of service for all Case Management programs

- Monitor client’s clinical/physical health and medications

PRINCIPAL ACCOUNTABILITIES

Completion of IRPs and other pertinent client documentation

Quality Case Management service for clients

Outside agency participation

Physical and clinical well-being of clients

EDUCATION, WORK EXPERIENCE AND JOB REQUIREMENTS

Education: Associate’s Degree in Rehabilitation, Social work, or related field preferred and/or 1 year related work experience.

Skill Requirement: Three months OJT required for efficient job performance.

Job Requirements: Excellent organizational skills and ability to prioritize tasks. Position requires great flexibility due to not having a set schedule or agenda.

COMMENTS
