Social Worker - Medical Group - Full Time

The Social Worker, Medical Group is a valued member of an interdisciplinary healthcare team, helping patients access the behavioral health support and resources they need to improve their overall well-being. This role provides an opportunity to make a meaningful impact by identifying, assessing, and supporting patients experiencing depression and other behavioral health concerns within the outpatient medical setting. In this position, you will conduct outreach to patients who screen positive for behavioral health conditions, complete comprehensive assessments and evidence-based screening tools, and provide brief interventions, psychoeducation, and supportive counseling. You will play a central role in coordinating care, connecting patients to community-based behavioral health services, and ensuring successful linkage through closed-loop referral processes and ongoing follow-up. Working closely with physicians, nurses, care managers, and community partners, you will help address barriers to care and promote positive health outcomes for a diverse patient population. Your expertise in behavioral health, resource navigation, and patient advocacy will support patients as they navigate complex healthcare and social service systems. The ideal candidate is a compassionate, patient-centered clinician who thrives in a collaborative environment and is committed to providing culturally responsive, equitable, and evidence-based care. This role offers the opportunity to practice at the top of your license while contributing to an integrated model of care that recognizes the connection between behavioral health and overall physical health. Additional responsibilities include documenting patient assessments, interventions, and care plans in the electronic medical record; assisting with urgent and emergent behavioral health situations; maintaining knowledge of community resources and managed care processes; and performing other duties consistent with professional licensure, education, and scope of practice. Through your work, you will help ensure that patients receive coordinated, high-quality care that supports their health, recovery, and long-term success.

This is a Monday - Friday, day shift position working 9:00 AM – 5:00 PM in the Ambulatory Care Network.

Preferred Criteria

  • Bilingual in Spanish
  • Familiarity with preventive care and knowledge of community resources
  • Experience working with individuals with behavioral health needs

Required Criteria

  • Master’s degree in social work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW)
