Speech Language Pathologist SLP PRN Home Health

We are seeking a PRN Speech Language Pathologist to serve patients in the Middlebury and Danbury area. You will have the opportunity to provide personalized care while working with a collaborative and supportive team.

What You’ll Do
- Evaluate patients and develop individualized plans of care.
- Provide speech and language therapy services according to physician orders.
- Help patients improve their communication, cognitive, and swallowing abilities.
- Educate patients and caregivers on home therapy programs.
- Maintain accurate and timely clinical documentation.
- Follow OASIS, PPS, and company guidelines.
- Collaborate with physicians and interdisciplinary team members to ensure continuity of care.
What We’re Looking For
- Master’s degree in Speech Language Pathology
- Current and unrestricted SLP license in the state of practice
- At least one year of clinical experience as a Speech Language Pathologist
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.
- Ability to travel throughout the Middlebury and Danbury coverage area
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
