Job Summary                                                                                                              

In collaboration with Maternal-Fetal Medicine (MFM) physicians and under the administrative supervision of the Maternal-Fetal Medicine service leadership, APN-Nurse Practitioner for Diabetes Obstetric provides exceptional care to a diverse patient population with gestational and pregestational diabetes in pregnancy within the high‑risk obstetric service. Promotes wellness and health maintenance by performing comprehensive diagnostic and therapeutic medical services. Provides diabetes education, insulin management, glucose monitoring oversight, and care coordination for patients receiving care in Maternal-Fetal Medicine and high‑risk obstetric clinics.

Develops and implements patient management plans in providing continuity of care. Manages diabetes in pregnancy within Maternal‑Fetal Medicine clinics and coordinates postpartum diabetes follow‑up and transition of care to primary care providers. Provides these activities throughout the Cook County Health (CCH) clinical sites. Maintains a patient tracking database and clinical templates for diabetic patients to support longitudinal care management and quality improvement initiatives within the MFM service.

Maintains professional and technical knowledge through continued educational programs and workshops supporting emerging healthcare trends. Serves as a clinical resource within the Maternal‑Fetal Medicine service and collaborates with MFM physicians, fellows, residents, nurses, dietitians, and other multidisciplinary staff to coordinate care for high‑risk obstetric patients.
Serves as a nursing resource, clinician, educates students, and participates in research and development. Coordinates procedures that require a multi-disciplinary approach. Performs duties within the legal guidelines of CCH policies.

Typical Duties

• Provides and manages direct patient care in areas of endocrinology and diabetes.
• Conducts patient’s health needs assessments
• Obtains comprehensive history and performs detailed physical examinations to determine accurate diagnosis
• Orders appropriate diagnostic tests or lab work necessary, i.e. x-ray, magnetic resonance imaging (MRI), endocrine tests, thyroid biopsies and dynamic endocrine tests, etc.
• Informs and effectively communicates with patient test results and/or diagnosis
• Identifies and communicates with supervisor any patient care issues; offers research guided recommendations for improvement initiatives if applicable
• Develops and implements patient management plans in providing continuity of care
• Performs procedures, treatments, quality improvement and administrative activities and other subspecialty dependent modalities within the scope of the supervising physician to improve patient outcomes
• Adheres to quality and patient safety standards set forth by CCH
• Adheres to infection control policies and protocols, i.e., hand hygiene
• Prescribes and dispenses medications utilizing best practices
• Complies with federal, state and local legal and professional requirements.
• Maintains professional licensing requirements and technical knowledge through continued educational programs and workshops supporting emerging healthcare trends

Minimum Qualifications

• Licensed as a Registered Professional Nurse in the State of Illinois required (Must provide proof at time of interview if already obtained)
• Licensed as an Advanced Practice Nurse-Nurse Practitioner in the State of Illinois or the ability to obtain prior to starting employment required (Must provide proof at time of interview if already obtained)
• Master’s degree or higher in Nursing from an accredited college or university required (Must provide official transcripts at time of interview)
• Certification by a nationally recognized body as designated by the rules and regulations of the Nurse Practice Act required (Must provide proof at time of interview if already obtained)
• Adult-Gerontology Acute Care NP (AGACNP), Adult-Gerontology Primary Care NP (AGPCNP), or Family Nurse Practitioner (FNP) certification required (Must provide proof at time of interview if already obtained)
• Experience in managing complex diabetes patients including pregnancy or other high-risk populations required
• Current Illinois Practice Act Prescriptive Authority required (Must provide proof at time of interview if already obtained)
• Must be able to travel work sites to provide outpatient care required 
• Valid Basic Life Support (BLS) certification from the American Heart Association required (Must provide proof at time of interview if already obtained)

Preferred Qualifications

• Doctoral degree from an accredited college or university preferred
• One (1) year or experience managing and/or monitoring complex diabetic patients as an Advanced Practice Nurse, including high-risk populations such as pregnancy, endocrinology, or primary care preferred
• Prior experience and knowledge in use of complex diabetes medical regimens including various types of insulin and all available oral diabetes and hypertension agents preferred
• Prior experience and knowledge of use and management of computerized databases preferred
• Certified Diabetes Care and Education Specialist (CDCES) preferred
• Completion of a one (1) year Cook County Health APRN fellowship preferred
• Prior experience managing gestational and pregestational diabetes in pregnancy preferred
• Experience providing patient education related to insulin administration, glucose monitoring, and diabetes self-management preferred
• Experience with diabetes technologies including continuous glucose monitors (CGMs) and insulin pumps preferred
• Experience working in a Maternal-Fetal Medicine or high-risk obstetric clinical setting preferred
• Experience providing care to reproductive-age or obstetric patient populations preferred
• Experience coordinating multidisciplinary care for high-risk obstetric patients preferred
