About this role
The Center for Human Development, (CHD) is seeking a HCWH (How Can We Help) Recovery Coach to join our community outreach team in Torrington, CT. Our residential and outreach staff establish life-changing relationships with our participants and provide opportunities to optimize their independence, rehabilitation, and recovery.
The HCWH Recovery Coach oversees a variety of cases through different venues on a daily basis in Torrington, CT and is able to provide those with an Opioid Disorder or recent Opioid Overdose seeking assistance the necessary tools to accomplish their goals. The Recovery Coach will maintain a schedule including but not limited to hours of actual street outreach, use of equipment and supplies as well as a schedule of external support providers to assist in the recovery from Opioid use.
RESPONSIBILITIES:
- Helps area Homeless Outreach Teams in locating homeless sites and aids in providing outreach alongside them
- Creates and maintains relationships with state, city and community agencies/resources including facilitating relationships with agencies not currently working with the Opioid Crisis
- Assists those with a history of Opioid use/Opioid overdose in acquiring services in the community
- Assumes hours of street outreach along with scheduled hours of appointment based outreach at community based sites (Soup Kitchen, Gathering Place, Charlotte Hungerford Hospital, etc.)
- Designs, promotes, and participates in activities and events that promote empowerment and recovery
- Maintaining records of Outreach usage by those with Opioid Use Disorders and/or Opioid overdose histories and their families
REQUIREMENTS:
- BA/BS Degree in human services or related preferred
- HS Diploma or GED required
- Two years experience working with adults with severe and prolonged mental illness or people in recovery preferred
- Two years in a human service organization preferred
- Valid driver's license required
- Registered & insured vehicle available for work purposes required
- Approved criminal background and driving check required
SCHEDULE:
1st shift, 8:30am-4:30pm. Days on: Monday, Tuesday, Wednesday, Thursday, Friday. Days off: Saturday, Sunday.
