Our House of Portland a program, apart of CAP Northwest is excited to announce that we are hiring skilled and team-oriented On-Call Relief Nurse. This position is responsible for performing the primary function of a Licensed Practical Nurse (LPN) by assisting in assessing, planning, implementing, and evaluating the care of Our House residents under the supervision of an RN and providing direct care to the residents as well as environmental care of the facility by performing the essential duties.

This is an On-Call position based at Our House of Portland in Southeast Portland. The selected candidate must be available for designated 24-hour on-call coverage periods and remain reachable by phone throughout their assigned availability. During on-call periods, employees are expected to respond promptly to calls or requests for coverage and, when needed, report to the worksite within the agreed-upon timeframe. Candidates must be able to arrive onsite within 30 minutes of being called in.

You will receive Availability Pay of $75.00 for each designated 24-hour on-call availability period, regardless of whether you are called into work.

If you are called in to work during your assigned availability period, you will receive your $75.00 Availability Pay in addition to your regular hourly rate for all hours worked.

Licensed Practical Nurse (LPN): $35.75 per hour
Registered Nurse (RN): $47.56 per hour

Who You Are

Current license in good standing to practice as an LPN in the state of Oregon.

CPR certified.

One year of relevant nursing experience

Must be able to pass a DHS background check.

Ability to cultivate and maintain professional relationships across a multicultural spectrum of organizations and professionals, including community partners and funders.

Successful experience working with ethnic, racial, economic, and sexually diverse populations and persons who have experienced houselessness, persons with a mental illness and/or substance use issues.

Experience working with people living with HIV/AIDS (PLWHA) and/or people with disabilities, chronic illness and/or other barriers to employment. A strong understanding of issues relevant to people living with HIV, communities of color, and the LBGTQ+ community and how those issues affect or influence service delivery.

What You’ll Do

Performs nursing procedures in accordance with licensure and skill level, utilizing principles of safety and infection control.

Provides assistance with activities of daily living.

Works in collaboration with resident’s primary care provider, clinical staff, and support system

Administers and documents medications and treatments using electronic medication administration records.

Contributes to the evaluation of the residents for appropriateness of care, medications, and treatments, as well as their response to the service plan.

Coordinates revisions of the service plan as needed and assists in implementing planned changes and activities to improve the quality of resident care.
