Schedule: Saturday & Sunday, 7:00 AM–5:30 PM
Work Location: Onsite

Job Summary

The Health Coordinator 1 supports patients by helping coordinate care and connecting them with the appropriate healthcare services and resources. This role works closely with RNs, Social Workers, providers, patients, and families to help ensure patients receive the support they need.

The ideal candidate is a critical thinker who can work independently, stay organized, and communicate effectively while also being a strong team player.

Key Responsibilities

  • Review patient information and reports to identify care needs and follow-up opportunities.
  • Coordinate patient care and connect patients with appropriate healthcare and community resources.
  • Work closely with RNs, Social Workers, providers, and other members of the care team.
  • Contact patients and families to provide information, schedule appointments, and assist with care needs.
  • Provide approved health education and support patients with their care plans.
  • Follow up with patients after hospital visits or transitions in care.
  • Document patient information accurately and keep care teams informed of important updates.
  • Address patient questions and concerns with compassion, professionalism, and excellent customer service.
  • Manage multiple priorities while maintaining accuracy and attention to detail.

  • Healthcare experience preferred.

  • Strong organization and time-management skills.
  • Ability to work effectively in a fast-paced environment.
  • Comfortable working with electronic medical records and computer systems.
  • Ability to collaborate effectively with RNs, Social Workers, and other healthcare professionals.

Qualifications

  • Possess a high school diploma, GED, or equivalent education
  • Have under (1) one year of relevant professional experience
