Overview
The Care Navigator, in partnership with the location leadership, represents Aveanna in business development activities, establishing a trust-centered relationship with physicians, hospitals, rehab facilities, specialty care clinics and other health-related institutions to develop a strong referral relationship for Aveanna’s services.The Care Navigator:Is responsible for general business development activity, communications and marketing support in a specified territory.Plays an integral role in the ongoing education of patients, hospital staff, physicians and community members.Ensures patient and family service expectations are met or exceeded with our referral sources. The Care Navigator drives revenue growth and partners with location management to ensure sustainable growth and contribution.
Territory: Southern FL Essential Job FunctionsDevelops business development strategy, goals, and plans which support achieving locations’ budgets; evaluate results and effectiveness of development activity. Support growth activities and establish strong relationships with new and existing referral sources.Participate in community education functions and events that directly impact or support the medically fragile pediatric and private duty community.Conduct ongoing meetings with physicians, clinic office managers, case managers and other applicable health professionals to build trusted relationships and understanding of Aveanna’s services.Monitor referral satisfaction to ensure Aveanna is a provider of choice.Provide feedback to location leadership and participate in creating solutions to address any concerns.Meet or exceed quarterly and annual business growth goals for specified territory.Maintain timely submission of call documentation, account contacts, patient referral documents, account profiles and expense reports.RequirementsBachelor’s degree in business, Healthcare, or related field; equivalent work experience may be substituted in lieu of degreeMust have a valid driver’s license and driving record that meet company standardsProficient in Microsoft suite of products including Outlook, Word and ExcelOther Skills/Abilities Embraces Healthy Organization culture and lives and promotes Aveanna’s Core Values.Highly motivated, results driven, and able to develop and build strong, lasting professional relationships.
