As a Licensed Practical Nurse, you are at the heart of what makes Active Home Care great. You will ease the minds of our clients' family by knowing their loved one is receiving the best of care. You will assist the Supervising RN with completing assessments on Brand new cases and routine cases.  As a LPN you are crucial to our success!

What you'll Do: Besides working with an AMAZING Team, here are some of the thing you will do:  

• Administer medication to consumer according to individual plan of care, physician's orders and emergent needs, and document clinical and progress notes on members' medical record.
• Perform assessments and observing and monitoring consumer's vital signs and responses to treatments and medications, reporting abnormalities or concerns.
• Coordinate care with other consumer care services (ie: Dietary, Pharmacy, Dental, Rehabilitation, Physical/Speech/Occupational Therapy, Audiology, DME).
• Assist consumer with daily functional activities, including but not limited to, hygiene, meals, ambulation and social activities.

 

Besides Having Fun and Loving our Consumers, Requirements include: 

• HS Diploma or Equivalent
• Current LPN License, Liability Insurance
• Min. 1 year experience in a licensed health care facility. 
• Prior experience working with the elderly and/or adults with disabilities preferred.
• This is a physical job, after all we are ACTIVE Home Care! As a result you will need to be able to:
  • Push, pull,squat, turn, pivot, transfer and  lift at least 50 pounds. 
  • Sit, Stand and/or Walk for extended period of time while providing physical assistance to consumer
  • And you may even dance when the music gets hoppin'!
